Yuvo works to increase access to primary care by supporting federally qualified health centers (FQHCs), the institutions that have dedicated the most to care for underserved communities. We work with FQHCs to help scale the quality of care they provide by providing access to new payment systems, developing and optimizing operational infrastructure, and convening partnerships in the broader care ecosystem so that they can increase their capacity and deliver the highest quality of care to their communities.


About the Role:

Yuvo is hiring a Chief Population Health Officer to lead our Population Health Team.

The Chief Population Health Officer (CPHO) is a crucial member of Yuvo’s executive leadership team and will lead critical functions that drive our performance across all our value-based care arrangements. Reporting directly to the CEO, the CPHO will work collaboratively with our internal teams and FQHC partners to establish and deploy successful population health programs across multiple functional areas, including but not limited to: risk adjustment, quality, and care management. To support our work, the CPHO will also be responsible for overseeing and scaling our diverse team of functional population health practitioners.

This is a full-time, 100% remote position for employees based in the United States.

Travel up to 25% across our markets (currently New York and Ohio) to work directly with field teams and FQHC provider partners as necessary.

Requirements

What You’ll Do:

  • Oversee Yuvo Health’s performance against rigorous value-based care quality and cost-of-care targets across all of our health plan contracts
  • Develop a library of best-in-class population health interventions and programs across multiple population types (e.g., Medicaid, Medicare, urban, rural, adult, pediatric, I/DD)
  • Establish and grow a best-in-class, scalable population health team to operationalize and deploy Yuvo’s population health interventions across our FQHC partner sites
  • Ensure that population health is adequately integrated with other functional areas of the organization (e.g., product, engineering, market, partnerships, operations)
  • Support our market teams to ensure a.) proper performance targets are established across our value-based care arrangements and b.) leadership buy-in of Yuvo’s population health capabilities
  • Work closely with our Chief Medical Officer and Chief Operating Officer to refine and operationalize Yuvo’s provider engagement and incentive strategy
  • Work collaboratively with our senior leadership team to make critical strategic and operating decisions on behalf of the organizations
  • Travel up to 25% to support field teams and interact with FQHC partners as reasonably required

What You’ll Bring:

  • 10+ years of experience in relevant population health expertise, either in a health plan or provider setting
  • 5+ years of direct management experience across population health functions (e.g., care management, risk adjustment, quality)
  • Deep understanding of managed care principles across all value-based care risk models
  • Knowledge of Medicaid regulations and standards – with a particular focus on quality (e.g., HEDIS) across multiple states
  • Expert team builder, having built teams from the ground up
  • Experience building remote first, centralized services teams
  • Comfort operating in a high-growth, early-stage organization and managing the ambiguity and messiness that can come with working in this environment
  • Proven data/metrics driven leader, having led organizations to consistent shared savings performance
  • Thrives in a collaborative, cross-functional team environment centered around diversity, acceptance, and belonging
  • A culture champion for the organization and a tireless advocate for your team

Benefits

Base Salary Range: $220,000 – $260,000
